Mechanism

Two halves that only work together:

  1. A locally calibrated trigger. Alert thresholds are derived from the city's own historical mortality-weather relationship (which air masses and temperature-duration combinations actually kill in this city), not from generic temperature cutoffs.
  2. A delivery chain the alert activates. Declaring a heat emergency switches on a set of services aimed at the people who die in heatwaves:
    • a public hotline run by a trusted aging-services nonprofit whose number older residents already know and call,
    • nurse triage on the line, with authority to dispatch,
    • field teams making home visits to callers and reported at-risk residents,
    • homeless outreach rules (in Philadelphia, Code Red) putting teams on the street,
    • block-captain buddy systems asking neighbors to check on isolated residents.

The design principle: a warning broadcast alone protects almost no one at high risk; mortality falls when the alert mechanically activates outreach capacity.

Where it fits

City scale. Best suited to cities that already have an established aging-services nonprofit with public trust, and a health department able to calibrate triggers and staff nurse triage. It complements, rather than replaces, registries and cooling centers by adding a demand channel (people call in) and a push channel (buddies and field teams).

Evidence

Philadelphia has run this model since 1995 as the first US Hot Weather-Health Watch/Warning System, with the Heatline operated by the Philadelphia Corporation for Aging. Ebi et al. 2004 estimated 117 lives saved in 1995-98 at a cost of about 210,000 dollars for the three years. Weinberger et al. 2018, a twenty-city comparative study, found Philadelphia was the only US city where heat alerts measurably reduced mortality, on the order of 45 lives per year. The system remains active (a heat emergency was declared in July 2026).

Implementation path

  1. Have health department analysts or academic partners calibrate alert triggers on local mortality data.
  2. Contract a trusted aging nonprofit to run the hotline, with city nurses embedded for triage.
  3. Pre-agree activation rules: what a declared heat emergency turns on (hotline hours, field teams, homeless outreach, media messaging).
  4. Recruit block captains or equivalent neighborhood figures for buddy check-ins.
  5. Evaluate against expected mortality each season and publish the results.

Trade-offs

  • The model depends on an intermediary organization that already has elderly residents' trust; a city standing up a new hotline brand from scratch should expect much lower call volumes.
  • Nurse staffing and field teams cost real money each alert episode, though the Philadelphia evidence suggests the cost per life saved is extremely low.
  • Hotlines are a demand channel: they reach people able and willing to call, so the buddy system and homeless outreach layers are needed to reach the rest.
  • Calibration is city-specific; copying another city's thresholds weakens the mortality link that makes the alerts credible.
